Dear patients
Following the recent announcement by the prime minister and as confirmed by chief dental officer, dental practices will be allowed to stay open from 1st January 2021.
During these unprecedented times we would like to thank you for your understanding and cooperation.
If we have arranged an appointment for you please read the "information leaflet" attached below before your visit. We would request you to sign and submit this information leaflet electronically to save time (you will receive a link via sms prior to your appointment).

Risks
Coming into any healthcare setting including a dental surgery may increase your risk of contracting COVID-19 as you will come into contact with members of staff and/or patients who could unknowingly be carrying coronavirus. Equally you could be infected, but not showing symptoms.
The surgery has put in lots of measures to reduce the risk to patients and staff including:
Reducing the number of people in our surgery to help prevent the spread to patients/staff
We have introduced social distancing in areas where we can
our practice follows the current national guidance
Individual decision
We recognise that, knowing these additional risks, you may not want to proceed with your appointment or treatment at this time. If you decide to delay your treatment, your dentist will inform you of the risks of not proceeding and any alternative treatments that they may be able to offer you.
Obviously if your symptoms or decision change please feel free to contact the practice and we can discuss all possible options.

If your answer to any of the following questions is yes, call the reception (01482320109) at the Whitehouse dental practice as soon as possible before you visit the practice
1. Have you tested positive for Covid19 in the last 10 days?
2. Are you waiting for a Covid19 test or the results?
3. Have you noticed any of the following symptoms in last 10 days -
New, continuous cough (A new, continuous cough means coughing for longer than an hour, or three or more coughing episodes in 24 hours. If the patient usually has a cough, it may be worse than usual)
High temperature or fever
Loss of, or change in, sense of smell or taste
4. Do you live with someone who has either tested positive for Covid19 or had symptoms in the last 10 days?
5. Have you been notified by NHS test and trace in the last 10 days that you are a contact of a patient who has tested positive for Covid19 and you do not live with that person?
If the patient answers NO to ALL they can be regarded as ASYMPTOMATIC. They can be treated at the practice If the patient answers YES to ANY they should be regarded as POSSIBLE/CONFIRMED COVID19. They shouldn’t be treated at the practice.
I confirm: that i have read and understand all above questions, and that my answer to all of the above questions is a “NO”.
I will notify the White House Dental Practice to change my/my child’s appointment if any symptoms of the virus develop after completion of this questionnaire
Please note following changes have been made in view of covid pandemic , as per guidelines
Personal belongings to be kept outside the surgery and kept to minimum
If you been told that you are part of “at risk” category or told to continue shielding by a doctor, please inform us as soon as possible.
Come alone to appointment unless accompanying a child or you are a carer
Please check for any updated covid info on the practice website
Please come to the practice, no more than 5 minutes prior to your appointment time. There is very limited sitting available in the waiting room due to social distancing guidelines, hence at times you might be requested to wait outside.
You can pay over the phone as well
Toilet facilities won’t be available.
It is compulsory to wear a facemask while in the practice
